Appointments
If you would like to make an appointment, please call our office at 718-692-1313. You can also fill out the Appointment Request form in the Contact section.

Insurance
As a courtesy, we will submit your insurance forms and apply any reimbursement to your account balance.

Cancellation
If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice. We ask for this advance notice so that we can offer this appointment to another patient.

Financial
All payment is due at the time that services are rendered. Forms of payment accepted by the office are check, cash, or any major credit card.
We understand that high quality dental treatment can be expensive. We are here to help. After extensive research, our office has partnered with Chase Financial Services. If the need arises we can help you obtain "Interest Free" financing to help pay for your dental treatment.
